Becoming a Caregiver in Arizona

It was not long ago that the term MMJ caregiver was unfamiliar to most Arizonians. But now that millions of state residents are licensed users of cannabis products, the role of caregiver has increased in both recognition and importance.

Caregivers are individuals that are well trusted by MMJ patients and ensure the patient receives everything they need for their daily treatments. Caregivers may help patients who wish to receive their medicines by smoking, vaping, edibles, or through applied oils. Marijuana patients often require daily treatment, so it is important to have a dependable caregiver on hand.

Some individuals have not pursued becoming a caregiver due to a mistaken belief the job requires  the completion of an extensive education program. Caregivers are not required to attend medical training but will need to know how to fill out the paperwork needed for their job.

Caregivers Provide Much-Needed Service

The Arizona Department of Human Services permits a single caregiver to provide services to as many as five MMJ patients. Each patient the caregiver assists needs to submit the appropriate paperwork to the state. The caregiver is also required to keep a copy of the card (in their possession) that allows them to provide services to each of their patients.

Once a patient decides you will be their caregiver, you must fill out and submit a Designated Caregiver Application. The application can be completed online and is provided by the Arizona Department of Health Services.

Prospective caregivers must submit the following information:

  • Full Name
  • Birth Date
  • Complete Address
  • Social Security Number
  • Recent Photograph
  • Copy of Government Issued Identification
  • Fingerprint Identification Form
  • MMJ Caregiver Attestation Form

Once you have submitted all information that is required of you, it will take about four to six weeks to receive an answer from the Arizona DHS. Accepted applicants will then receive a caregiver card that identifies them as the caregiver for their patient.
